Oddly, even though these things take less than an amp, the manufacturer's instructions also want them GFCI protected. I know Rinnai and Noritz are that way.

Not that I haven't done it.
I've always done mine with an MC whip into a switch in a handy box. I have generally powered it from a basement receptacle circuit, which is normally GFCI protected by some means already.
 
I have not connected any of the instaneous boilers or furnaces before, but the ones I have seen come with a cord and plug factory installed.

Scary! A 30A OCPD feeding a 15A cord that plugs into a shower head. No way legal or UL. If the required bathroom receptacle is GFCI, that helps, but there is no guarantee. 406.8.b.c says no receptacles in shower stalls so this could be legal.
