At first glance, looking at the dashed line and associated contact points, I wonder if this is actually showing that the primary is six windings that can be connected in series for 12000V input or in parallel for 6000V input?
If not, then the only possible tap changing would be for one set of windings shown to be many fewer turns than the other allowing for only two different tap positions with the ratio change not shown on the nameplate. Essentially connecting the two windings (per phase) in series or leaving one set disconnected and taking the ouput from the other set only.
Or the dashed line and contacts actually hide the presence of an unspecified number of taps at different turn counts at the end of each of the windings, with the mechanical changing happening through an insulated mechanism to the outside.
If so, it would be an internal tap changer with the tap points not actually leaving the transformer. (Which makes a lot of sense for MV/HV.)
There has to be better documentation than the nameplate available somewhere, even if you have to go back to GE with the serial number.
