Can you use phase A and hi leg phase b with a two pole breaker to power an appliance like a 208/220 water heater? Or should the b phase only be used with three pole applications?

Yes you can use 2 poles in a Delta. You just cannot have any single pole ckts originate on the B phase. The resultant voltage would be 208 not 120.

Just be sure to use a straight rated breaker (240) and not a slash rated breaker (120/240). See 240.85 more more info.

To a line-to-line (aka phase-to-phase) load, the voltage-to-ground is not relevant.
