Transformer disconnects

where can i find where on the LOAD side of the transformer there is suppose to be a disconnected within 25ft of the transformer for that load?? Or something that talks about the 25ft. thanks.

There is no requirement for a transformer disconnect, but in many cases there is a requirement for secondary conductor protection. Look at 240.21(C).
