Can someone point me to the article or code section that requires a disco for a transformer? I can not find this anywhere. Is this a requirement or not?

A transformer is not required to have a disconnect.

Many times a fused disconnect is placed on the secondary side to provide overcurrent protection to the secondary conductors.

I believe Section 490.22 applies to transformers operating over 600V. Note it doesn't necessarily require a disconnect, but it does require an isolating means.
