Should the primary and secondary grounds and neutrals be connected in this 480v/208v transformer?

The bare copper is part of the footer ground

There should not be a primary neutral, but assuming the system bonding jumper is at the transformer, everything else named in the original post will be connected together at the transformer. If the system bonding jumper is at the first OCPD, the neutral will be connected to earth there and not at the transformer.

system connecting to earth, is it 250.4(A)(1)? what I usually see in high rise building that contractor connect the wire from floor transformer to building steel. in my opinion the wire should be taken to a basement where main utility service and main grounding electrode. is it acceptable what contractor is doing?

Yes, connecting to the local building steel in a high rise steel framed building is acceptable.
