Transformer grounding

I received the following correction notice for wiring on a new 45KVA 480-120/208 V. 3-P 4-W located in a new office building 2nd floor. ?Cold water ground (copper pipe) is O.K. for bonding X-form but XO needs to be grounded to building steel?. Inspector indicated the cw ground was not sufficient because of the valves and couplings; it is necessary to have a bond around them because of the joints. Why is this necessary?

The 2nd correction for same X-form was ?only one wire per ground lug is allowed?. Two #6 ground wires were installed in a #14-2 ground lug. I know about neutrals but never heard this before .As a side note, the Inspector took pictures of the installation because he liked the work!
 
Read 250.30(A)(7) (1) and (2) and the exceptions. The water pipe probably doesn't make it. I don't know what he means by it would be ok for bonding the transformer.

Read the last sentence in 110.14 (A) for the answer to your lug question.
