When applying 450.3 (B) and using both primary and secondary protection, is there a specified maximum distance from the transformer that the secondary OCP can be installed?

Welcome to the forum.
Not when you are talking strictly "transformer protection". The secondary conductors from the transformer (which also require overcurrent protectiion) will have a length limitation in most cases. start at 240.21(C).
