Transformer primary switches

I am wondering if you have a bottom fed transformer primary load interrupter switch that would be front access only how would you pull the incoming cables to the top lugs. I've seen some switches such as Eaton's MVS and Square D's HVL which mount the switch in the center of the enclosure. In this case I believe the incoming line would be routed behind the switch and somehow pulled and connected to the lugs at the top. I 'm just wondering how you could get the cables to the top of this type of switch with only front access but I know it has been done before.

Bottom feed gear I have seen provides lugs at the bottom and is hard bused to the switch. can't think of another safe method without lots of room for cable routing.
