is the transformer secondary overcurrent protective device required to be located in or on the transformer enclosure?
 
No, there is no such requirement.

It is very common for a MCB in a panelboard to servce as the transformer secondary overcurrent protection.

No.
See 240.21(C) for protection of transformer secondary conductors.
Most installations seem to fall within 10' and 25' limits.
