Transformer Secondary Protection

I am installing a 45kva 3phase 120/240v delta to 120/208y transformer.
The transformer will be installed in the electrical room approximately 120ft from the load it will serve. The circuit breaker feeding the transformer will act as the Primary disconnect/OCPD. The secondary feed will terminate in a 3phase fused disconnect that will feed an elevator motor. The elevator room is 120ft away.

Does the circuit breaker on the Primary side need to be able to be locked out?

Do I need an additional disconnect means/OCPD for the Secondary at the transformer location?

No, and yes, in that order. Check out 240.21(C). You need overcurrent protection for the secondary conductors. You don't meet the requirements for protecting them with the primary OCPD. You also don't meet the requirements for protecting them with an OCPD at the end of the run. So you will need a secondary OCPD within the electric room.
