Transformer secondry disconect

I am installing a 225 KVA 480delta pri 480 Y secondary trans. Do I need a disconect at the trans. on the secondary side. It is over 25 ft total length. The secondary conductors alone are probally 65 ft long. And in sizing the Diconect on the primary side, the trans. is oversized so do i use the rating of the trans. or the rating of the connected load. It is feeding a CT in a hospital. The specs say it needs to be at least a 160 kva trans. Thanks
 
Table 450.3 for the OC requirements; then read all of 240.21 (C) (1), (2) & (3). Unless you are in an industrial installation (240.21 C 3), your OCP for the secondary should be within 10' of the XFRMR. For the primary side use the full value of the installed transformer.
