Are 5kva single phase transformers sold with a nameplate primary of 120/208v and a secondary at 480v?
When you have the option, look for a delta primary. You should be able to find a 208D-to-277Y/480 unit.

As mentioned, 1ph or 3ph?

hello
I have only come across the 208 single-phase primary transformer in my workstation. I think that is the only possible outcome as we deal with all types of transformers. But am inquiring with fellow technicians and engineers to see if 120/208 primary is a possibility. ON information about transformers https://www.pcbway.com/blog/Enginee...al_connector_in_the_transformer_industry.html will be a helpful article too in the meantime.

Not if you mean supplying a single, center-tapped primary with three conductors (line, neutral, and line) from a 3-phase source.

It would be easy to make a transformer with 120v and 208v taps on a single primary, but you would supply it with only two wires.

I have had trouble locating one. Any suggestions?
You would never have a three wire 120/208V primary transformer winding, because the neutral is not needed. You would use the two wire 208V primaries you have found. This is similar to how you do not find motors that need 3 wires for single phase.

Is possible to build one with taps for 120 and 208. The 120 tap will not be a mid point tap like it is on a typical 240 volt coil.

Is not physically possible with single phase only system. 120 and 208 in a three phase wye system exist because of the phase angles in said three phase system, but you are talking about a single coil for the primary of your single transformer and mid point of 208 is only 104 with a 180 degree angle between the outer points. If you had a 208 volt coil with a 120 tap in it, the voltage from that tap to other end of coil would only be 88 volts.

I will add it is possible to build an open delta secondary with two phases and neutral for primary input, but this is a three phase system and has other than 180 degree phase angles involved. You were wanting a single phase transformer, possibly with such input and that physically is not possible.
