If it's 3 phase Is the formula KVA/1.73xvoltage? If it's single phase you just exclude the 1.73?

what determined if it's a continuos load(3 hours or more)? The panel loads? Thanks

Yes, the nature of the load, however, in most cases your transformer primary overcurrent protection/conductor is going the be sized at 125% of KVA or larger due to (a) inrush current and (b) the allowances of 450.3(b).
