Transition to Finished Space

What is the best practice to transition MC cable from inside a wall into a finished space (where it will be surface mounted and then hard-wired to a HW tank). Is a flush-mounted or surface mounted j-box required? Can I just penetrate the wallboard? Thanks,

Use a nail on box, mud ring, finish cover with ko. From ko whip to wtr heater

When you come out of ko, can use a 90, so can attach to wall before you connect to WH

Do you need a disconnect? If so, that's a good place to transition.
In one of the back KOs and out one of the bottom KOs.
