Two 200 amp subpanels & 10ft tap rule??

I have a 400 amp disconnect. I need to run a 60 ft. run of 3/0 from this disconnect to a 200 amp main breaker load center. From this same 400 amp disconnect, I need to run a 70 ft run of 3/0 to another 200 amp main breaker load center.

Does code require I set two 200 amp disconnects within 10 ft. of the 400 amp disconnect and then make my runs from them? OR can I just come directly off the 400 amp disconnect?

I am a little fuzzy on this. 10 ft. tap rule is giving me trouble understanding????
 
The only tap rule that would allow you to run over 25ft would be the "outside tap rule" in 240.21(B)(5)
 
Your two 200 amp disconnects could use the 25' tap rule.

In plain English, can I do it without the two 200 amp discounts? I get confused trying to interpret code. Does "outside" mean outdoors or just "beyond" ? This is all inside a commercial building, subject to strict code inspection.
