Does anyone have pictures of a handhole for a 200 amp underground service?

Do you usually run pipes into sides or from the bottom?

I stub up from bottom with 90's. Extend up to 6" below lid. Duct seal ends. Put 4" pea gravel in bottom. Using 90s makes pulling easier
Typically its an 18 x22 x 12 deep conc hh with galv lid. For service conductors, our poco has special size, I have never installed.

There are many styles, sizes, applications of handholes. First would be to determine the general conditions of use. Is it in an area where traffic or load bearing is needed? What size of conductors to be inside and determine minimum size for bending radius, point A to point B for pipe minimum spacing? What is the water table for the area? Will additional drainage be needed? (Will make a difference for conduit sealing to prevent water intrusion.) Will it need to be concrete encased? Is extra durability to frost/freeze expansion needed? (The one shape shown below would be more resistant to heaving vs some others.) Etc.
With the info provided unable to just say here is one. There are many styles, shapes, and sizes. Open bottom, side feed, bottom feed, variety of load bearing capability, etc. Once you know your criteria for installation Google can be your friend. Word of caution, there are some boxes out there that are not designed for electrical installations, don't know if they would be acceptable if doing an enclosure within an enclosure installation like I've seen to prevent water intrusion.
Here is one sample of a handhole:
