Underground Vault Grounding

What does NEC Code say about grounding the cover lid of an underground vault that houses communications wires & fibers and electrical power conductors (240Vac)? We have followed NEC Code 800.52(A)(2).

What does the Code say about a metal door frame in a building? In my opinion, if it is likely to become energized, you are required to bond it but if not, you are permitted to ignore it. :D

How would you ground bond in a vault? One of the idea was to ground the cover lid to the ground conductor. Would that be acceptable bonding?

dvuong, if this is a CEV for a telephone company, the hatch is bonded to the MGB via # 6 AWG conductor and terminated to the hatch with a 2-hole terminal using 1/4" bolts, nuts, flats, and lock.

No, this is a regular utility vault.

The L&I inspector would like us to isolate the power conductors from the communications wires and ground bonding the metal cover lid. We are planning to isolate the power conductors from communications wires by using flexible tubing. Do you have any idea of how to run a grounding wire from the lid to the isolated grounding conductor in the flexible tube for bonding?
