Undisconectable power in building envelope

Can anyone steer me in the right article that deals with a utility termination can and running nondisconectable power through building envelope. My first thought was to treat it like a fire pump and encase it in concrete but I would like to find the article and be clear.
 
IMHO the critical distinction is not whether the power can be disconnected, but rather whether it has any kind of OverCurrent Protection Device (OCPD) sized to protect the wiring.

If it is service wiring, the code limits the distance that such wires can run inside a building.
The wording (OCPD which effectively ends the service wires located as close as possible to the point of entry into the building) leaves some room for AHJ interpretation, but typically ranges from no distance at all to 5' or 10'.
But if they are encased in 2" or more of concrete, they can be considered to be outside the building.
