US Forms for recording Insulation Tests

Interestingly, there does not seem to be a standard form that is intuitive for recording test data.

I threw this one together within a few minutes, but I am curious what I am forgetting, or if there is something already out there (as I cannot find one here in the US :happysad:).

I can find them for 3 phase Euro systems everywhere (as they are required for ANY installation there, prior to service connect, but not here... :huh:)

Any suggestions? Ideas? or just a plain reference to a good form? Thanks!

What are the last two columns?
Why a 10 min test on 600 volt wire?, I have never seen that.
What test is LN-G?
Why do you think LN-G and L-N is a destructive test?

DAR and PI ratios, PI is usually only done on transformers and motors.

Honestly any insulation test can be destructive.. I have fixed the form to reflect that.. Thank you for the note

My thought was at first he last two can be severely destructive if voltages are used over rating voltage (ie someone forgot to remove an LED light, which would not allow a standard ohm test for continuity...)
This is why I test first at or below operating voltage... (kind of a personal note, reminding me not to screw it up...) but this really applies to all of them...

The LN-G is mainly for circuits in a live installation.. (naturally with the tested circuit disconnected and locked out)

Thank you methinks, I am looking for suggestions to improve it so anyone who picks it up can use it and clearly understand anything recorded.

I am thinking that there are a lot of very knowledgeable people here, with a lot more experience than me, who could have some useful input...

The goal is to have a really good form anyone can use, interpret and share with others who might need it...

ok, so I tried to incorporate suggestions and some other stuff.. and began to make it more eye friendly with some shading... :cool:

one thing that bothers me is size... it has to be printed on 8.5x14 Legal size paper to be easy to use.. otherwise it's SUPER small and annoying to look at... Had to add 12 columns to get corrected values on it.. not sure if this is the best way to go.. :blink:

Looks very much like a standard UK EIC (Electrical Installation Certificate) or for an existing installation EICR (Electrical Installation Condition Report).

I did posted a copy up many moons ago and got in to a major argument with Iwire over it.
