Most utilities are going to be more concerned with what your load profile looks like then the setting of the overcurrent device.
If you have 400 amp overcurrent device but only 250 amps of load (that runs continuously for certain time period) they likley will not put in more then a 225 kVA bank, composed of 75 kVA single phase transformers. If you have short periods of higher load, they may still go with the 225 figuring it has time to cool between peak loads. If future load is expected, they may go larger right away though.
