It wasn?t ?by? Mike Holt. It had the name of another person, presumably the creator of the spreadsheet, in the description on Mike?s Free Stuff page. I do not know why it was removed from his web site.
I managed to download a copy before it was taken off Mike?s web site. Since it is not my property and not (I believe) in the public domain, I cannot offer to send anyone a copy. Perhaps if enough people send in requests that it be put back in the Free Stuff page, Mike will comply. But this Forum is not the way to communicate that request to Mike.
Please note: I believe that it has one error, not in the math but in the instructions for using the spreadsheet. If it does get posted again, I will look to see if that error had been corrected.
