Walker Floor duct parts??

I have a customer that would like a floor outlet cover replaced and I can't seem to find any info on it can someone please help me identify the brand for sure and maybe a part number?? I think it is a Walker floor system, but the adapter plate doesn't look like anything you can get today, and the concrete ring is quite narrow. This was originally installed around 1996.

I have looked at wiremold but they don't seem to have anything that looks like it. It seems like they changed all the designs from what used to be available from Walker, most everything is a wider floor plate. I even looked on ebay to find a used one.
