They can remove all the insulation from all the conductors and it still won't be much chance of failing in that manner. Something got hot, most likely the conductor termination point. I also agree with others that those bones are arc chute remnants.Did you guys miss the BONES in the picture? I say RATS caused the arc. Plastic got burned to produce carbon, carbon started conducting at some point and finished the job long after the skin disappeared.
