What do you think caused this?

They can remove all the insulation from all the conductors and it still won't be much chance of failing in that manner. Something got hot, most likely the conductor termination point. I also agree with others that those bones are arc chute remnants.

Unless Micky Mouse got crossed with an ostrich, those "bones" are too long to belong to any rodent.
 
This is what it should have looked like before the meltdown. Maybe that helps with the debris question.
