What is "exposed" wiring

On a job where the specs call for all exposed low voltage wiring in an attic to be in conduit.

NEC seems to define exposed as "on or attached to the surface"

Would you call wires completely under the insulation as exposed??

If it was inside a wall cavity it would not be exsposed, your example would be considered exsposed

Yes I would because they're not protected by a raceway, so in this case they are exposed to
Physical damage.
