What is typically the operation to access an underground easement utility primary line if the pad mounted transformer is far away from it?

For example, if you have an underground easement running and you try to tap it - do you need a a pad mounted transformer nearby it?
what type of accessible junction box do they use if the building utility transformer (for example 150' away) is not nearby the underground easement pathway. I'm just trying to get a rough idea how the connection between the underground easement and a transformer that is far away is being connected directly on the primary side.

Thank you

I dont understand what you are asking or trying to do. You want access to the utility primary? Or are you just curious how/where POCO would tap into the system? If he latter I say they would want to come off a loop feed bushing or add a loop feed insert in an existing transformer. Maybe they would come into a new pad mount switch or junction if a transformer was far away.

The latter. You answered my question. The nearest existing transformer is very far away, so i guess a new pad mount switch and or junction would be the solution. I was trying to figure out how they did it if there was no existing transformer nearby. Thank you for the response!

Although an underground splice is possible, I have the feeling most POCOs wouldn't do it that way.

Edit: I mean they could make a splice with connectors in a vault or hand hole - what I meant was they probably wouldn't direct bury a splice.
