What issues do you see with this?

Long story short but there was a lost pipe in the wall so we had to chisel it out to find it. He foreman wanted to just use the hole in the wall as a I box and out a 12x12 cover over it? Says it is a non combustable wall so is the same as using a handhold? Something doesn't seem right to me so shoot away!

I guess technically it is not a listed box or a handhole. However, if you don't have any other option I don't see any safety issue with using that as a junction box. Yes it hack work but it was not done intentionally. If you do use it as a J box, make sure the front metal cover is bonded to the EGC.

Can you install two 90 degree elbows facing outward towards the opening then install a surface mounted 12x12 box on the wall. I think I would do this if I could. Another words the 90 degree elbows penetrate the back of the box.
 
How did you guys find the conduit behind the cinder block wall??
 
Well it was a 400 foot home run to some motorized shades/vents. The rope broke so I pushed a fish tape down the wall and it only went about 10 feet which was about a foot up from the floor. 7 #10 xhhw solid... Not fun. My suggestion was to take a 8x8 box and cut it in half horizontally, then drill holes for the pipes coming in and then overlap the sides of the 8x8 and slide it up tell they both touch the block on the top and bottom and then hammer it in and ground the cover....
