Either conductor pulled so tight it damaged insulation at the "turn" inside the LB, or a conductor was pinched under the cover?

That is soot from the flue of a power washer surrounding the LB. Users are supposed to roll it out the door before starting the boiler.

At least it wasn't PVC conduit - have a customer with a hog barn that has properly supported conduits and they are severely drooping in the room where the power washer is supposed to be stored, been that way for 20+ years, this happened when the building was in it's first year of use.

They learned it needs to be rolled out the door:blink:

One of the quickest diagnosis I’ve ever made. That’s about 8” inside the door and was as far as I needed to go. Phase to ground was the trigger along with a phase to phase that is in the low megaohms.

You win.

The cause was the the heat generated from boiler of the power washer melting the insulation of the THHN. Result was the grounded phase.
