Here is a scenario. You show up to a job. Panel has a space left open no breaker, or a KO left open. Your tasks includes work performed with in the panel.

You're the electrician:
Assuming the open spaces are not result of work you're performing.
Do you get a filler and plug up the hole?
Or this wasn't your work and leave it?

You're the inspector:
Do you call it out?
Do you hold up giving a final as a result of missing filler?
Let it slide as it wasn't a directly connected issue to the job done that you're inspecting?
 
Electrician: Leave it better than how it was or at least bring it to someone's attention. Fix it and charge for your service.

Inspector: Point out the issue, recommend the solution and don't get in the way. Its the owners responsibility,
 
AS an electrician, if I had what was needed on sire I would fix it and mention I had done so, If I didn't have what I needed to address the issue I would ask the customer if he wanted me to take care of it (extra trip, etc).
As an inspector, if it wasn't in the electricians scope of work I would notify the owner the repair was needed,
 
I have a hard time believing any electrician would not have filled in the hole.

IMO it is not the responsibility of the guy hired to do something to fix anything else. Whether it is the job of the inspector to point out existing code violations not related to the permit is something I don't know, but I doubt it is.
 
Breaker fillers cost almost as much as a breaker.
I have some 1" fillers (Eaton BR) in my accessory box. If that doesn't fit, I'll put a breaker in.

I carry blunder plugs in my accessory box.
Lots of 1/2" & 3/4" and even a few larger ones.
I'd bet money I have it and I'll put it in

Blunder plugs...😅 never heard them called that before.
