When was equipment ground fault protection for services 1000 amps or larger required? I find a 2008 NEC book on NFPA Link that shows it as being required. I'm doing a Power Study and I need to pass judgment on a 480V, 1200-amp service that doesn't have it.

Well, it is pre 1975 as that is far back as I go. But keep in mind that not all "1000 Amp" services require GFEP. For example, if you have a 1000 amp panel board with 6, 200 amp mains then GFPE is not required. Of course that does not apply to an install today as the 2020 does not allow this type of service arrangement.

Hasn't it also always been required for solidly grounded wye systems operating over 150 volts to ground Current wording says and less than 1000 phase to phase, though I could see it possibly once saying 600 volts.

So delta systems in general do not require this, neither does 208/120 Wye systems.

Correct. And , yes, it was 600 volt until the NEC raised most things to 1000 volt.
