Wiring Three Pole with 5-15R_6-15R

Hello All,

I am new to this great forum; thanks for the opportunity to exchange info and learn! Could I please get your thoughts if anything in the NEC or otherwise wisdom pops with regard to the following scenarios wiring three poles to a 5-15R & 6-15R duplex outlet:
