work clearance in front of a junction box especially above a ceiling

What rule in the code governs how much clearance you need in front of and/or around a standard junction box? Or for that matter any sized pull or junction box?

I don't think there is any special requirement for working space around junction boxes.

(A) Working Space. Working space for equipment operating
at 600 volts, nominal, or less to ground and likely to
require examination, adjustment, servicing, or maintenance
while energized
shall comply with the dimensions of
110.26(A)(1), (A)(2), and (A)(3) or as required or permitted
elsewhere in this Code.

If it is "likely" that you will have to work on it while energized it needs the working space. if it is not "likely" you don't need the working space.

The fact is that if it's above the ceiling you can never comply with the working space so the point is moot even if someone thought that is was required.

It does need to be accessible, but they don't say who it needs to be accessible to. There are things that I once used to be able to gain access to that I can no longer get to. Crawl spaces that are one CMU block high are no longer on my list of spaces I will crawl into, my luck is I would get in but not be able to get back out.
