Does article 110 pertain to mechanical starters or controllers? If it does, how can you legally install a starter for a air handler unit located in a ceiling? What if the starter is integral with the equipment?

Art 110 is a "General" Article and applies to every install unless s specifically alter by another Article.
You work still need to comply with the working clearances in 110.26 but therer is a Exception in 404.8 which will allow the switch (controller) to be mounted near the equipment if the equipment is in tre air
