Equipment grounding conductor ran with supply conductors to a separate structure

When installing a 100A subpanel to a separate structure, is an equipment grounding conductor needed with the supply conductors if there is a ground rod installed at the separate structure?

Yes, see 250.32(B). The neutral is not bonded to the GEC or loadcenter so without an EGC there is nothing to clear a fault.

Will this create a ground loop if the EGC is connected on both ends?

No, it's the same as connecting an EGC to any subpanel. Remember the grounded conductor is not bonded to the EGC or GES at the detached structure, it floats so there is no loop possible.
