Does a 200A Service with a 200A Disconnect installing 4/0 SER from the disconnect to the panel need to be derated ?

Read 310.15(B)(7) and take special note of how it defines "Main Power Feeder".

IMO, no it does not if it qualifies as the main power feeder that Roger was talking about. Nowhere in Table or Article 310.15(B)(7) does it state that de-rating shall be considered.
