Inquiring minds want to know what happened? Besides turning into expensive scrap metal.
A little more background.
The kit had been in storage for about seven years.
Storage being in a mangrove swamp area with no control over ambient conditions. Anti-condensation heaters were not energised and there was no ingress protection.
Fried snake anyone?

How many do you want?
Incorrect connections.
Loose connections.
Foreign bodies in the kit.
Excessive moisture.
Wrong voltage.
Lightning strike.
Act of god.
A screwdriver left in the wrong place.
Equipment failure.
A rat across the bars - it happens.
Wrong toss of the dice....

Yes. Lightning strike, Act of god can not be detected by any precommissioning tests. But others could be.
