If you have so little 'clearance space' in a box that you can't get a push-in blunder plug to work, the box is overfilled.

Not always necessarily true.
Sometimes it can a challenge to get at a KO that is located in the back of an electrical panel that is loaded on that side.

Doesn't everyone know you're supposed to use 4" sq covers for large KOs?

Nothing wrong with that and I almost exclusively do that for anything over 2" as I don't normally stock any plugs larger than 2".

Used a bolt and fender washers occasionally on small holes when I maybe didn't have a plug handy.

Had a couple irrigation control panels this past year that had plastic hole plugs that failed and then mice got into the panel - they really make a mess of things when they have an offseason to do their thing in there. I will not use plastic plugs in those applications anymore and may even replace existing ones when I encounter them. These were factory installed in openings for options that were not used that happened to fail in the incidents I was referring to:rant:

Not in all situations anyway.

I use them quite often indoors where there is not any extreme conditions expected. Outdoors you have cold, heat, UV at very least working against them.

I like the 3 piece ko plugs. Also will use the hoffman as a lot of what I install is NEMA 4, also Hoffman comes in SS. I kept an assortment of hoffmans on my truck
