what can and cant be installed before a Service disconnect?!

So I seem to have a lot of confusion on this topic and I'm hoping to understand this subject better.... I see a lot of questions on what can be installed before a service disconnect... In my area, I see a lot of double throws or automatic transfer switches installed under a meter housing on utility pole, and then ran into the house to the service disconnect. There is usually a generator set and wired to the genset side of the automatic transfer switches, or there plug installed on the generator side of the double throw so they can take portable genset out to the transfer switch. I'm wondering if this is legal, or should there be a service disconnect before the ats or double throw? or does the ats or double throw have to be labeled service equipment, service rated, or some other type of listing? any and all help is always appreciated! Thanks in advance!
 
okay so as long as the transfer switch is service rated it can go after the meter and before the service disconnect. what if it comes from the secondary side of the utility xmfr? also wondering if service entrance rated means the same thing as service rated? So in that case if I were to install a service rated solid neutral double throw or ats after my meter in an existing service, it become my service disconnect. I would bond grounded and grounding conductors at the ats or double throw. I would then have to run and E.G.C. inside to the existing service disconnect "main panel in the house" so that I could isolate the grounded and grounding conductors at the existing main panel which is actually considered a sub... I'd also isolate my grounded and grounding conductors at the genset because of it being a non separately derived system....
