Meter Main on pedestal 125' from a house. Does the service inside the house need a main breaker or will a main lug suffice?

If this is a new installation the separate structure (the house) will require a disconnecting means, a grounding electrode system, and a 4-wire feeder from the pedestal. The disconnecting means can be a main breaker in the panel.

2017 cycle and the panel is adjacent to the j box on the side of the house where the URD changes to SER.

Keep in mind that the meter has a 200a breaker built in.

That doesn't matter. You still need a disconnect at the point of entry

Don't get overcurrent protection and disconnecting means confuse with one another.

Yes you have overcurrent protection at the meter main, but, you still need a disconnect at the "structure" or house.
