Seeking replacement cover for older Square D load center

I don't want to say it has to be replaced, since the only thing wrong is the cover:

12.5" x19.5", Square D. The model number is completely unreadable.
Could someone help with a model number, series, or source for a new cover?
 
Wow! A QO meter main, that is a real blast from the past, might be best to get a sheet metal shop to fab a new one, as that is a rare item.
 
On some the farms around here if you probe around in the dirt nearby you might find the cover just below the surface. It been laying there long enough it got covered in dirt, dust, even has grass/weeds growing on top of it.

Otherwise metal fabrication is probably the best bet.
