Main breaker for Milbank combo panel.

Why not? Sounds like a MLO panel and he wants to install a 200A branch. As long as there is no specific restriction listed on the thing whats the problem? Its probably a Siemens Q2200B
It would have to be reconfigured differently from how it is put together at the factory 🤷‍♂️

It comes with 1+3 bridged and 5+7 bridged

It would need 1+5 and 3+7

Maybe use short pieces of #2 copper, effectively paralleling out from breaker to some Polaris taps to get back to single conductors to feed new panel.

No you have the wrong breaker. The correct one (pretty sure I had the right part number) is a branch breaker. The one you showed is a replacement for a factory 200A main breaker.
