PVC & Manhole

I've been trying to google pics of pvc duct bank going into a concrete manhole for a pic of the inside and can't find anything good.

When pvc conduits terminate within manholes:
1. Is there usually some some of connector, end bell or bushing on end? Some of the pics I seen( not clear at all) look like it's just the endof the conduit cut flush with the manhole wall.
2. Most times are there "window opening" or just holes for each conduit?

Bell ends are often used with PVC. If there is a square window in the vault they get chopped out, the raceway installed with a bell end, and then the hole gets grouted closed.

Thanks. Guess RMC could also have end bell? Would think probably more likely a grounding bushing would be used?? Spec dictates

Typically a bonding bushing for RMC.
