That cover is some 40 years obsolete.
What is wrong that you want a replacement one?

Many people just a something made at a sheet metal shop.

Nobody makes panels with breakers mounted upside-down any more.

How difficult would it be to replace the panel, or even just the guts?

been looking at that option too, but nothing matches size of existing enclosure.
I'm having a custom cover made using another as template, hoping my steel guy can work some magic!
When I've done it, I mount the guts first, align the cover to the guts, then make new holes in the old box to match the new cover, or make new holes in the new cover to match the old box.

I am still trying to picture what is wrong with the original cover in the OP.

You can make a flat cover and shim the interior out to meet it.

I am not saying a flat cover will work. It won't unless you shim it out Someone made the original cover. I worked at one of the largest sheet metal contractors and metal fab shops in MA. They could easily make that cover.

A residential duct contractor can't do it you need the right shop. A shop with a plasma cutter can easily make that including indenting the cover so it will fit.

If you want to make a flat cover and shim the guts you can do that as well.

The other way is to have a flat cover made and cut the panel edge on 4 sides
