The 125 volt is rated for a maximum voltage of 125 volts. One ungrounded conductor, one grounded conductor, and one equipment grounding conductor. The 250 volt is rated for a maximum voltage of 250 volts. Two ungrounded conductors and one equipment grounding conductor. The old two wire receptacles don’t have a equipment grounding conductor. The difference is slot shape, which is basically a rejection feature.
