Out here in So. California, I had seen an old Building in Long Beach (or might have been San Pedro...???), which was still being fed via a 2? system, along with a "Newer" 3? open Delta Vee 4 wire (common system for a few Buildings).
Also, at an Auction, there was a whole bunch of 2? 4 Wire Stainless EXO Fusable Disconnect Switches, NEMA type 1 Subpanels, plus quite a few Motors.

This was in early 1980's. Since then, no such Animal(s) seen anywhere.

A couple of "P.S." items for FYI (Acronym attack!!! :) ...)

Although the above description is not 100% Technically correct, it scratches the surface, so to speak.
The 1? Motor might have an offset that comes close to 90?, but the 2? system is very much going to have a solid 90? offset.

2:
There are still 2? Motors being used for Stepper / Servo Motor applications. Along with those comes the 2? clock circuitry and related control circuitry.
