This code says in 210-4 "In dwelling units, a multiwire branch circuit supplying more than one device or equipment on the same yoke shall be provided with a means to disconnect simultaneously all ungrounded conductors..."

Did this at some point become a requirement for separate circuits on the same yoke?

Yes, 210.7(B) in the 2005 NEC.
Yes, but this is a commercial kitchen. Does the phrase "in dwelling units" still apply? .

In 2002, 210.4 stated that multiwire circuits could be considered to be multiple circuits. Then, in 210.4(B), it went on to state that in dwelling units, you were to disconnect all conductors that fed the same yoke.

But at the same time, in 2002, 210.7(B) stated that any multiple circuits on the same yoke were to de disconnected at the same time. This could lead to interpretation issues, as a multiwire was permitted to be considered a single or multiple circuit, so even outside of dwelling units, it could be considered as required to disconnect all ungrounded conductors.

In 2005, they have cleared it up by dropping the "in dwelling units" language from 210.4. Now, whether it's in a dwelling unit or not, it's required to disconnect any ungrounded conductors on the same yoke simultaneously. :)

We should all just work from the 1993 code for this forum. Things were so much simpler then.
