Does the panel in your own home have absolutely nothing infringing the work space requirements at all times?
Yes. It's on an exterior wall. Code aside, in some of those restaurants I visited it might take them an hour to reset a tripped breaker.
 
You just can't recess them 3 inches in from the finish wall surface;)

Code violation or not, I have to wonder why a restaurant owner/manager would want to have such a downtime, then find it will not reset so they have wasted an hour before even finding out they need to call a second party for service.

It also bases those spaces on an average sized person I would assume anyway, even though some may appreciate even more space in some circumstances, others are maybe just small people and simply don't need all the space the code gives them.
 
The Receptacle I have behind my refrigerator is one of the most easily accessible one that I've got.
I'd much rather roll the fridge out of the way to get to that one rather than have to battle all of the brooms,buckets,mops,canned foods,winter clothes,and Vacuum Cleaners my wife has piled in our pantry. :)
