Good that you already saw that. If it says it, it says it, puts you in the clear.

It's entirely possible that the day they needed the service panel, the only thing available off-the-shelf had the higher rated breaker and that was cheaper than flying out something else. Then they back-filled the documentation to show what was in place. Happens all the time and there is nothing wrong with it.

That seems to be the most likely scenario. The utility available fault, and the 42KAIC are indeed clouded with delta 1. D1 just says "Rev", I usually try to put "plan check", "client rev" etc. to clarify, but in any case the scenario you described is most likely. I think I'm safe putting a 10K subpanel keep in mind it will be several hundred feet away. After all is said and done, I think I may just replace the 200A CB with a new one that does have a double lug option and let the rest simply be constructed as shown on plans.
