It's hard to include estimate cost and time for hammering out codes.

IMO it would be better for all if it were clear.

I'm with ya,

Probably would be best if you and I penciled in our own row that stated, or, "residential foot traffic only, not subject to vehicular traffic".

Then it would make perfect sense to an old school guy like me.

Nothing in code is ever clear - all of it is subject to change if someone submits convincing enough changes for the CMP's to accept the change.

That said you either submit estimate for what needs to be done or eat costs if you were wrong and forced to change it.

If not going to be inspected - submit estimate accordingly based on fact that competition may cut corners.

Without the driveway - installing RMC only 6" deep could actually cost less then direct buried UF cable if digging 18-24 inches deep is a problem, some think they are limited to UF cable for some reason in such installations.
 
Although row 6 is lacking the word under it is very clear what it actually means. The entire section 300.5 is for underground installations.

but did they intend it to mean under a paved driveway/parking area or any driveway/parking area?

All the other 18 inch allowances in that column have concrete or other protective layer involved. The non dwelling related street, road, driveway actually is supposed to be deeper even though it may also have similar protective layer.
