You obviously have field wiring above the designed line 110..3(B)
The existing lugs and the ones you propose appear to violate 110.14(A)
Don't see any grounding electrode conductor.
What is the length of your secondary tap to your OCP device ?

Are you referring to the 25' rule ? if so well within. I believe it is less than 5'
